By signing or submitting any prescription slip (RX slip) or its equivalent to DTek Dental Lab, you acknowledge and agree to comply with all terms, conditions, and policies set forth in this agreement. This includes, but is not limited to, our liability limitations, product usage guidelines, and quality standards.
We encourage all dental professionals to carefully review our policies before submitting any prescriptions to ensure a smooth and effective partnership. By working with us, you affirm your commitment to upholding the standards that help deliver the best outcomes for your patients.
2. Limitation of Liability
At DTek Dental Lab, we are committed to providing the highest quality products and services to support dental professionals and their patients. However, please note that DTek Dental Lab shall not be liable for any incidental or consequential damages, including but not limited to inconvenience, lost wages, chair time, or pain and suffering, that may arise as a result of using our products or services.
While we strive to ensure the best outcomes, certain factors outside our control may affect results. We recommend that all dental professionals exercise their judgment and expertise in evaluating and applying our products.
3. Payment Terms
- Invoice Payment:
- To ensure a seamless experience with DTek Dental Lab, we require that all invoices be paid in full on or before the 10th day of the month following the month in which the invoice was prepared. This applies to all cases, even those not yet seated in patients’ mouths. We appreciate your prompt attention to these payment terms, as it helps us maintain the quality of service and product delivery you expect.
- Please ensure that your payments are made within the specified time frame to avoid any disruptions to services or late fees.
- Payment Options:
- Credit Card on File / Autopay:
- Autopay Option: For your convenience, you may choose to keep a credit card on file with DTek Dental Lab. If you opt for this service, your account will automatically be charged on the 15th of each month for any outstanding balances. This ensures timely payment and helps streamline the billing process, so you can focus on your practice and patient care.
- Non-Autopay Option: If you prefer not to have your account set to autopay, please ensure that your balance is paid in full before the 15th of each month. This will help you avoid any delays or additional fees while maintaining smooth service and uninterrupted delivery of products..
- No Credit Card on File:
- If you decide not to keep a credit card on file, please ensure that your balance is paid on or before the 20th of each month. Failure to meet this deadline may result in your account being placed on hold, which could impact the timely processing and delivery of your cases.
- Credit Card on File / Autopay:
- We recommend setting reminders to avoid disruptions in service and to ensure a smooth working relationship with DTek Dental Lab.
- Late Payment and Account Hold Policies
To ensure consistent service and timely delivery of products, the following payment terms apply to all accounts:
- Any unpaid amounts will incur a 1.5% monthly interest charge.
- Accounts with unpaid balances after the 20th of the month will be placed on hold until full payment is received. This may affect the processing and delivery of future orders.
- Accounts remaining on hold for 60 days will be referred to our collection service, and all outstanding balances will incur an additional 25% service fee.
We encourage you to make payments promptly to avoid any disruption to your services and additional fees. Your cooperation is appreciated in maintaining a smooth and efficient relationship with DTek Dental Lab.
4. Warranty
- Zirconia, PFM, Emax, and All-Metal Fixed Restorations: 5-year guarantee from the date of delivery.
- Removable Restorations (including dentures, flexible, and alloy partial dentures): 2-year service warranty from the date of delivery.
- Temporary Appliances & Repairs: Warranted against defects for 60 days due to the higher incidence of damage.
- Custom Implant Abutments: Lifetime warranty.
5. Turnaround Times
- Turnaround times mentioned on our website (DTekdentallab.com), platforms, or marketing materials are averages.
6. Prosthesis Insertion and Patient Maintenance
- All prostheses must be inserted by a licensed practicing dentist.
- Patients must adhere to a semi-annual dental maintenance program (cleaning and examination) at the office of a licensed practicing dentist.
7. Warranty Exclusions
The warranty will be void under the following conditions:
- Impression Issues:
The following cases are excluded from warranty coverage:
- Unclear Impressions: Cases with unclear impressions where the technician must proceed without a new impression.
- Margin Trimming: Cases where the doctor requests the technician to trim margins “best they can” due to unclear margins.
- Double-Sided Quadrant Impressions: Cases with double-sided quadrant (triple tray) impressions showing tray sides through the impression material.
- Inlay/Onlay Cases: Inlay or onlay cases recorded with a double-sided quadrant (triple tray) impression.
- Putty Wash Technique: Impressions sent using the putty wash technique.
- Alginate Impressions: Cases with alginate impressions that are not already poured upon submission.
- Remake Cases:
- Original Prosthetic and Model: Remake cases must include the original dental prosthetic and model work at the time of the remake to avoid additional charges.
- Differing Instructions: Remake cases with instructions differing from the original case (e.g., shade, material, type of clasps for partials) will be fully charged.
- Shade Remakes: For shade remakes due to DTek Dental Lab’s error, the case will be refabricated at no charge, provided the remake uses the exact same shade as originally requested. Any changes in shade will result in full charges.
- Impressions and Models: Any remake case without new impressions and/or models will incur additional charges.
- Remake Limits: Any case can be remade at no charge under the conditions above only twice. The third remake and any subsequent remakes will be charged in full.
- Digital Remakes: For digital remake cases, the original restoration and models must be received by DTek Dental Lab within 7 business days of the remake request. Otherwise, the case will be fully recharged.
